This information does not apply if one is a psychiatric mental health nurse practitioner. Many southern states do not recognize nurse practitioners as independent providers and will often reject the claim even if the NP is a preferred provider in his/her own state. I have learned this the hard way.

Of course. Our contact’s name is Robert Anthony and he is a Health Insurance Specialist in the Office of E-Health Standards and Services at CMS. We asked him the question: “Would offering the patient the clinical summary after every visit count as providing a clinical summary for this measure?” Robert’s response was: “Yes, as long as the offer is made to each patient. However, just posting a sign saying that a summary is available would not be sufficient to meet the measure”

Hope this helps!

Can you please clarify this comment with some supporting documentation? Either a link referring this online or the name and title of your contact at CMS?
Thanks.

Thank you for your comment. That is a really great question, and sadly, it is one that does not have a definitive answer. It would be a Herculean task to research every American health insurance company to determine if they 'allow' E&M codes from psychiatrists. However, we can say that the major insurance payers in the US, for example the BCBS Association, United Health Group, Aetna, and Cigna do not have blanket exclusions restricting coverage of E&M codes for psychiatrists. An M.D. or D.O. of any specialty is able to use E&M codes as long as they are providing services in line with the requirements of the chosen E&M code, and properly document the visit to back up the use of the E&M code. Check out our best billing practices blog for information about collecting insurance coverage information through benefits checks prior to providing services - it's always a good idea to check benefits to determine any coverage restrictions a patient's insurance plan might have before seeing your patient.

Are all insurance companies allowing psychiatrists to use E&M codes. i know that the APA Protests Insurers' Discrimination in Paying for E/M Services. An article was written in the Psychiatric News on April 1, 2011. What is the outcome?

Deb,

Authorizations are tracked (current # of authorizations - 1) when an appointment is submitted through the calendar. Also, if an authorization applies only to certain CPT codes, you can specify that in the authorization information.

Dr. Charuvastra,

It's very humbling for me that my colleagues are my customers and even more humbling when they reach out to express satisfaction with the EMR or our great staff. Thank you very much for your comment.

Let me know if you are coming to Seattle or going to the APA in Hawaii. It would be great to connect.
